Perfil metabolómico cutáneo en inflamación persistente de origen multifactorial

El estudio analiza la inflamación cutánea persistente de origen multifactorial, caracterizada por la interacción entre disfunción de la barrera epidérmica, alteraciones metabolómicas, disbiosis microbiana y activación inmunológica sostenida. El objetivo fue examinar el perfil metabolómico cutáneo y su relación con variables biofísicas, inmunológicas y microbiológicas. Se aplicó un enfoque cuantitativo con diseño no experimental de alcance descriptivo-explicativo, basado en la revisión de informes de organismos internacionales y literatura científica (2021–2023), utilizando correlación de Pearson, regresión lineal múltiple, alfa de Cronbach y prueba de Shapiro-Wilk. Los resultados evidenciaron una firma metabolómica diferencial entre piel sana, no lesional y lesional, con alteraciones en lípidos y aminoácidos; se identificó una relación inversa significativa entre la pérdida transepidérmica de agua y la hidratación cutánea, así como una asociación directa entre la disbiosis cutánea, el predominio de Staphylococcus aureus y la severidad clínica; además, se determinó la activación del eje NAD+/NAMPT/PARP1 como mecanismo central en la persistencia inflamatoria y la capacidad de terapias dirigidas para restaurar la funcionalidad epidérmica. Se concluye que el perfil metabolómico cutáneo constituye un eje integrador para comprender la inflamación persistente y orientar estrategias de medicina de precisión en dermatología.
